Information
Enclose an initial deposit of $500.00 per student.
Six months before your course, send us a final deposit of $1395.00
per student and $200.00 per guest, which will pay your tuition
in full and $300.00 (plus guest deposit) toward your resort portion.
Thirty days prior to the course, you must pay for your hotel/dive
package and extra nights, tax and gratuities. You will then need
to pay only your incidentals (food, bar, Nitrox and other purchases)
upon departure. Make checks payable to Cathy Church.

Liability Statement
By reserving in this course, I agree that Cathy Church is
not responsible for any actions of any airline, Sunset House Hotel,
Sunset Divers or any other business or individual whose services
I may use in relation to my taking a photo course.
As a healthy, certified diver, I realize that
I am knowingly and willingly engaging in a potentially dangerous
sport. I understand that Cathy Church is my photo instructor only,
not my dive master or scuba instructor, and that all diving is
conducted under the direction of the resort dive master. I will
refuse to participate in any dive or activity that I judge to
be unsafe. Cathy Church and her staff are not in any way responsible
for any accidents that might happen to me while traveling to and
from, or during my stay in the Cayman Islands. I am solely responsible
for my preparation for (including but not limited to equipment
configuration, adequate rest and hydration) and execution of the
dive (including but not limited to monitoring my equipment, depth,
time, gas mixes and personal limitations).
